Many insurance policies do offer some coverage for Vision Therapy. While most insurance policies do not cover all of the costs, many do pay a portion of the cost. It is up to you to contact your insurance company to determine what coverage is available according to your policy.

We will provide you with diagnosis and procedure codes at the consultation for you to give to your insurance company to obtain an estimate of coverage.

CLAIMS

How will my insurance claims be filed?

You are responsible for filing all insurance claims. Minnesota Vision Therapy will supply you with a statement after each session to assist you in the claim filing process. The statement will include the diagnosis codes and all other applicable information the insurance company will need. You will need to contact your insurance company to determine how to properly file a claim. 

Is Minnesota Vision Therapy Center In Network or Out of Network?

Minnesota Vision Therapy Center is an out of network provider for all insurance coverage. Out of Network Coverage is usually paid at a different rate than In Network Coverage. We are not under any contract or discount program with any insurance provider. You are responsible for full payment of services and you will receive reimbursement from your insurance company. If your insurance provider sends checks to Minnesota Vision Therapy Center we will send you a check for the amount as soon as possible.

APPEALS

When do I file an appeal?

You are able to file for an appeal once your claim(s) have been filed and you are denied coverage.

In order to start an appeal process with your insurance company you will need an official "Denial Letter" from your insurance company including the reasons for denial of coverage. Please check with your insurance carrier to determine how you file for an appeal. If you choose to file an appeal please bring your official denial letter to the front desk for further assistance. We are not able to assist with the appeal process unless you have an official Denial Letter.
